Thank You for your order! Your Order Form enables us to track the receipt of your records, insure we have all your records compiled and finally to load your records on your password protected Flash Drive.

Please make sure that you completed the Authorization Form for each medical provider you are requesting records, print the form and either fax or mail to your provider so that they can fax your records to our secure server.

Also make sure that you go to our Products and Pricing page to choose the Flash Drive and services you desire.  We offer several options such as adding a Living Will or Medical Power of Attorney to your Flash Drive.  Your password-protected Flash Drive can hold up to 1Gb of data so there is no need to worry about storage capacity.  Just think what information you might need to have with you in an emergency.

When your Flash Drive is ready we will send it to you by U.S. Mail, along with a wallet card, so you can record your personal password that you provided on your order form. 

The wallet card also has space for your personal information, emergency contact data, current medications and allergies.

We will also send you an email to inform you when your password protected Flash Drive has been mailed and a reminder of your selected personal password.

We also want to remind you that for each friend that purchases a My Medical History Online Flash Drive and lists you as the person that made the referral, we will credit the account that you used to purchase your Flash Drive, $5.00 as a thank you.

After you receive your medical history on your password protected Flash Drive and your friends order from us, just contact us at the email address below with their name and address and we will credit your account.
