  1. Why do I need my medical records for myself?

If you're like most people, you may have records from multiple sources, including your primary care physician, the emergency department, imaging centers, outpatient laboratories, and different hospitals.  Currently, there is no simple and accurate way to ensure that all of these records are assembled in one location.  Even your own physician may not have all the information.  In the case of an emergency, records of your medical history, medications, allergies, and prior tests can be life-saving.  Despite continued amazing advances in medical care, it's an unfortunate fact that physicians and hospitals are inexcusably still using 1970's technology for the creation and control of medical records.  Taking control of your own records is one way to improve this situation. 

  1. How do I get my medical records?

Simply print a copy of our AUTHORIZATION FOR RELEASE OF MEDICAL RECORDS, fill in your name, address, phone number, and social security number, then sign and date the form and fax or mail it to any healthcare providers, hospitals, urgent care centers, or emergency departments from which you've received care in the last 1-2 years.  They will send the requested information directly to our secure fax number, where it is converted into an electronic format for storage on a flash-drive.

  1. Are there any other costs?

    Hospitals are increasingly charging for access to medical records, but usually only in cases where actual photocopies are made.  The cost for copies can be around 89 cents per page, but since we rely on faxed transmission, the costs are typically waived.  Please check with your particular provider or hospital before making your request. 

     

  2. Is it secure?

The transmission of your records is as secure as your email or on-line banking.  Your records go through a secure server with SSL encryption and are then moved to a password-protected flash drive which is mailed to you through the U.S. Mail.  Only you can read the contents using your selected password.  No records are saved on our site.

  1. How do I pay?

    Fill out a "Contact Information" form on our Contact Us page, and provide your email address, after you fax or mail your AUTHORIZATION FOR RELEASE OF MEDICAL RECORDS.  Once we've received your records, we'll contact you via email to ensure that we've received all expected records.  At that point, a simple click on the Buy Now button our Services page will let you make any form of payment you wish. 

     

  2. Who is behind MyMedicalHistoryOnline? 

We are a small family business, started by a retired school principal, John C. Mazurek, who wanted to find a better way for people to organize a very important aspect of their life after experiencing the difficulties in doing so himself.  John's family helps out with the  business; his wife is a retired medical insurance company employee, one daughter is a registered nurse, the other is an emergency room physician, one son-in-law is an emergency room physician, and the other is in law school. 

  1. Is MyMedicalHistoryOnline any different from other groups storing medical records online?

    Yes!  Unlike other organizations, we do the work of collecting your records for you, then give you the result.  Other companies (Microsoft and Google being two of the largest) are attempting to facilitate the storage of medical records on-line (but still have a long way to go), and many smaller companies offer a system that requires you to obtain all the data and upload it yourself, but with MyMedicalHistoryOnline.com, we do the hard work, then you control the data, and determine exactly who has access to it.  

     

  2. Is my privacy protected?

Yes!  We take privacy very seriously. Our privacy policy is very simple: We do not provide your information to anyone else. Period. Your order information is kept only as long as is needed for us to send you your records. We communicate with you via email only as needed to update you and confirm any questions; we will never pester you with unsolicited email, and after you’ve received your records, you won’t hear from us unless you contact us first. We only need your preferred mailing address for your flash drive shipment, and do not need your phone number at all.
