1. Enter your data on our secure, password protected My Medical History Online form.

2. Print out our AUTHORIZATION FOR RELEASE OF MEDICAL RECORDS, fill out the form, sign it, and send it to your healthcare provider by fax or mail.

3. If you have records at other locations, (hospitals, imaging centers, etc.) send them an authorization for release of medical records as well.

4. If you need additional information about our services use our Contact Us page. 

5. When your records are received, we'll send you an email to let you know your electronic file on Flash Drive is ready to be shipped.  Simply pay through our convenient Services page.
